James Warren Pike Essay question: How far and in what ways has a writer presented justice as an important concept? Refer in detail to any work of literature you have studied. Justice is one of the fundamental principles upon which builds a nation. V for Vendetta conveys justice as fairness. V for Vendetta is a film based on a graphic novel written by Alan Moore and David Lloyd in 1982. Through the plot and motifs presented in the film, we gain insights into the idea of justice. Several characters in the story bring the juxtaposing views of V? action, while V sets the conflict between fascism and anarchism. The director, James McTeigue leaves V? s act of justice in the film unjustified, and guides the audience to interpret it themselves. The opening scenes of the film accentuate justice through a voice over, this voiceover not only retells the history of Guy Fawkes, but also sheds light onto the revisionism of V. V as the protagonist in the film, is critical to the representation of justice, because the film portrays the definition of justice through V? s actions. The Guy Fawkes mask depicts the main adumbration of justice in the film. Guy Fawkes was involved in the Gunpowder Plot in 1605. The Gunpowder plot was a group of Catholics attempting to blow up the Houses of Parliament. V? s impulsion in the film is to destroy the central criminal court, the Old Bailey. V wears a Guy Fawkes mask throughout the film, not only to hide his identity, but also to bequeath the idea of being a superhero to the audience. The Guy Fawkes mask is not a V? s privileged weapon. In the scenes of the Shadow Gallery, the point of view shot of V wearing his mask gives the audience the feeling of wearing the masks themselves. V also shares the masks with the public and invites them to attend the 5th November protest. The mask is a representation of political justice. These scenes suggest to the audience that all people should have the right to fight for political justice. This is also supported through the history allusion of the Gunpowder plot, where Guy Fawkes was brave enough to stand up against the government. The anonymous V attempts to annihilate the Norsefire dictatorship by killing several members of the party. James McTeigue illustrates the British government as fascist through the naming the different bodies of the government. „The Eyes? are referred to the surveillance organizations, â€Å"The Nose? to the detectives, â€Å"The Mouth† to the propaganda and â€Å" The Finger† to the fingermen. V kills â€Å" The voice of London†, Lewis Protherol. Protherol? s rant on V suggests to the public that no on escapes judgement, which is irony to himself getting killed after. On one hand, it was an act of personal justice, because Protherol was a commander in the Larkhill detention camp. On the other hand, Protherol was one the main propaganda creators, a part of â€Å"The Mouth†. The motif of fire strengthens the idea of justice in the scenes of Larkhill. This fire was first introduced in the opening scenes of V. A fiery, sharp blaze swoops across the screen, drawing the letter of „V?. The fire symbolizes destruction, anger, and also passion. The destruction was the violent acts V was going to do, anger was V being tortured in Larkhill, passion was V enjoying the violent acts. V for Vendetta allows audience to justify the morality of V? action and to identify the British government as a functional institution. The story of Larkhill is told to the audience through the quest of unearthing V? s past. Detective Finch investigates Dr. Surridge? s diary and reveals to the audience how V was taught to obey orders through violence. The same way V pursues justice by killing people who were involved in the Larkhill detention camp. Through the voice of V, the speech given by V on British Televison Network further advocates the idea that all people should have the right to fight for political justice. V? s speech is also invigorated through a piece of artwork he had in the shadow gallery – A mirror with carved Latin words „Vi Veri Vniversum Vivus Vici? This not only suits V? s love of words starting with the letter V, but also empowers the power of truth. V? s speech on British Television Network accuses the public of bowing down to the High Chancellor Sutler, allowing fear to be the government? s biggest weapon. V fought for fairness because he though „People should not be afraid of their government, the government should be afraid of their people? V alleged the government of oppressing the people, and justice should be attained through the power of the people. This justice he is suggesting to the people is not violence, but is the power of the truth. V destroys the Old Bailey not only to get attention but also to prove to the public how vulnerable the government may be. Scenes on the roof with Evey, with the background music of parts of the 1812 Tchaiskovsky, suggest to the audience that V was enjoying it and treated it as a musical performance. On the other hand, he broadcasted the music to the public through the public surveillance speakers.

Belonging Essay

Belonging is the finest thing in the world that knows how to belong to oneself. You have to belong to yourself before you belong to society. Belonging is a quite a hard concept, as you can argue that people who don’t want to belong actually â€Å"belong† to a group of people who don’t want to belong. But what is belonging? One idea is that you belong when you feel comfortable with people who have similar objectives, goals, and aims as you. But, as i realised while analysing a film in fact it’s for this unit, if you don’t feel comfortable with the group you believe you belong to, then even if you have the same goals for example cricket. Even though everyone’s goal is to win, should the team not befriend you, make you feel like you are excluded from the team, then you don’t really feel that you belong to the team so it’s basically what belonging is about. My first text is about a song by my island home by Christina Anu and how the singer portrays aspects of belonging through her song lyrics. Firstly she explores the idea that belonging to people or places shapes our identity and then she explores how the barriers which prevent belonging can create a need to belong. The song Island Home shares her perception that belonging to certain people or place creates our ideology of life and where we rightfully belong. We live by the sea and My home is Australia this use of language shows pride and belonging to Australia. This creates and brings belong into place where our perspective of belonging to be one of happiness. Belonging is evident in the song in the boat on the sea again, and I holding that long turtle spear, and I feel close to where it must be. The composer emphasises the point that her home must be by the sea because that’s where she is from and where she feels that she belongs, the pride of her belonging. We create our own identity from the feelings and affections we have with belonging to certain societies and their way of life. Inclusion and isolation demonstrates our personal identity which portrays who we are. Our perspectives and views are changed depending upon what we belong to. My home is Australia; we are a land bound by sea to show that Australia is a part of her which will never be lost is also shown as her the composer is from a native background (aboriginal). She emphasises on how her and land are one, and it is this affection that had shaped her identity. Belonging in the song My Island Home? which I find is people who had experienced belonging to a place or community. But the song conveys the composer as; these glimpses are obviously not enough to make you feel the same way real belonging makes others feel. The composer shows the barriers she has to overcome like modernisation, family and living far away from the place she belongs to and this belonging is felt by her. The composer shows the importance of belonging by using a reflection question to compare her life in the present time and when she belonged. Will this place ever satisfy me this shows how much she wants to belong again, which portrays her strong love for her place of belonging. The composer only reveals through her dreams how she feels towards belonging. Six years I have lived in the desert, every night i dreamt of the sea shows the identity of the composer since Australia is covered in desert lands ad this shows the connection she has with Australia through her. My second text is on Edward scissor hand by Tim Burton, I was actually going to Life of Pie by Ang Lee but it was quite tough to relate to belong so I went with Edward scissor hands plus I got a bit help my English teacher for this movie so it make my life a bit easier. At the beginning of the film we see that Edward lives alone in a gothic castle. He has been isolated in the castle for an indeterminate time. Below the castle is a small town a close knit community and barely visits the castle, not knowing that Edward lives there. Peg feels sorry for him and decides to take him back to live with her in the town. At first Edward is being treated well because he is a great novelty. No one has ever seen someone like him before and he quickly becomes a celebrity. He is embraced by the townspeople; they think he is very useful with his scissors (haircutting, gardening etc. ) one of the town women thinks he is sexy and tries to seduce him. He refuses and runs away. This incident becomes the catalyst for Edwards’s rejection and alienation from the community. Edwards’s personality consists of innocent, naive, limited social skills, very gentle and very experienced at relationships with people, no idea of sex, manners etc. Other than what his creator taught him from books. He is very trusting until he finds himself he victim of a setup. After things become worse, Edward finds he is a victim and cannot remain in the community. Techniques used to convey the context relevant to belonging is that Edwards’s clothes and appearance is a complete contrast to town’s people. When he first arrives Peg him clothes to wear which he puts on over his own costume. This helps him develop a sense of belonging to the family and the community and reduces his difference to the townspeople. However the fact that he doesn’t remove his clothes indicates that he wants to maintain his identity. Later in the film he becomes angry and feelings of rejection and alienation by the town cause him to rip off. Music is used in a number of ways to show Edwards innocence and purity. His character is linked to Kims in the way music and lighting are used, the haunting they’re music, together with soft/bright lighting plus show plus the slow motion of the camera reinforces the innocence and the nature of their love for each other. At the end of the film, when it becomes impossible for Kim and Edward to belong to each other in a proper relationship. He goes back into the castle and she to the town. All though they cannot belong together physically they will always belong together emotionally. This is shown at the end of the film as Edward continues to make snow from the ice and flashback to the narrator at the child’s bedside. The picture book, The Island portrays belonging as impossible if there is an ignorant and fearful dominant culture. The book portrays to the responder into almost alienation and new comer into a strongly built and well organized society. The alien is portrayed as thin and white-white and unclothed showing that the new comer is an empty canvas to be painted opinions onto. The new comer is also thin and innocent looking showing how the character is reliant on acceptance to survive. Whilst the dominant society is given the appearance of large, over bearing, they are always portrayed as a collective group portraying their dominance. They are in black clothing, they all appear to look the same- again representing there collectiveness, and they are holding pitch forks portraying their feelings towards the new comer unwelcomed. The dominant society belong together they appear to look the same, and have much of the same views, they have collectively created a community that are dependent on each other and there likeness. The society is well established and fearful of anything that will imbalance its structure and that is different from their social, cultural and historical similarities. The new being does not offer any change, it does not do anything to radicalize the community but just its presence of being different offers physiological fear to the dominant society. The book demonstrates the effect of fear, what it does to a community and the object being feared. Through fear of this unknown the community alienated the unknown, trying to forget its existence when this did not work the community they contemplated integrating the being. But the overbearing fear of the effect the being would have on their system led them to cast out the different being. The being that was submitted to the dominant cultures fear was treated as inferior, become the dominant society did not want the unknown to belong they create a barrier around themselves in fear of the unknown and it defining themselves as exclusive. Because the dominant society was afraid of the unknown they showed ignorance and a lack of open thinking which creates difficulties to them from acceptance and understanding of anything other than their own people. Therefore Belonging is a way of life which characterises us humans and creates our individuality. Due to the complex and abstract nature of the concept of belonging, a true sense of belonging can be found in different circumstances for different people. As each individual has their own desires, needs and values, they find their place in the world and a genuine sense of belonging in various avenues. Many individuals find the strongest sense of belonging through relationships, due to the fact that by nature these connections fulfil the human need for social interaction and enrich the lives of the persons involved.

Assessment and Accountability in Nursing Mentors have responsibility to assist students to develop their clinical skills and knowledge on placements. As well as developing clinical skills, students also develop their interpersonal skills and as a result of this experience gain maturity. (NMC, 2008) .To determine students have achieved the professional standards or competence, mentors must make assessments while their placement. This is vital to protect the public from unsafe and incompetent practitioners. Assessment is an essential role of a mentor. A good assessment process gives the students valuable feedback, it helps them to identify where they are, highlights, deficits in their learning ,identifies what they need to do and enables them to set realistic future goals.(Danny Walsh 2010). A good assessment process help us to predict future behaviour of the nurse we train. However we can cut down the risks by ensuring that the assessment processes itself meets certain standards and criteria which are considered best practi ce. Proper assessments of students are essential because, they are the future nurses are responsible for protecting the public in patient care, (Necklin and Kenworthy, 2000:108).Through assessment mentor can identify the strength and weakness, knowledge and practical skill of a student. According to (Walsh 2010) student nurses are assessed for variety of reasons in their clinical practice. Examples of this; to monitor the students’ progress to give feedback, to discover learning needs, to encourage students, to monitor their progress, to assess students level of competence, to assess knowledge, skills and attitudes, to measure the effectiveness of the teaching and to safeguard the patient and protect the public. As students work alongside with mentors, need to encourage students to perform patient care through instructions, guidance and supervision. While they are caring the patients mentors can observe and give feedback, this means while student is learning she is being asse ssed. Managing the process of assessment and feedback is hugely important. Students fail their placements for a wide range of reasons, but certain key factors can help minimise this happening. The student needs to feel welcomed and wanted to be able to perform into the placement quickly and so get on with meeting their competencies. Duffy (2003) cited several reasons for students failing and the key aspect was the lack of a good mentor/student relationship. Mentors must be meeting with students regularly can identify the problems or deficits the students going through. Learning needs should then be identified and action plan must be put in place. Mentors and assessors have the right and responsibility of making professional judgements about the performance of their student. (ENB and department of Health, 2001).Students needs to be respected while maintaining their professional standards, it is important to recognise the rights and to be supported to succeed in clinical practice. Mon itoring progress is about finding the students quality and quantity of learning and any difficulties students may experiencing that can be identified and action plan to be applied. There are different methods of assessment like observation of patient care, questions and answers, reflection, briefing, self assessment peer assessment, testimonies, portfolios.(Stuart 2003). Duffy and Hardicre (2007) identify the reasons of a failing student are lack of insight, lack of interest poor communication or interpersonal skills, personal issues, being over confident. The Nursing and Midwifery Council (NMC2004a) states that nurses are accountable for their own practice. The public has a right to expect competence from the qualified professional in healthcare. During the operation we monitored blood pressure, pulse rate, and record abnormal symptoms such as abdominal pain, nausea, vomiting and pain score of the procedure was assessed at the end of the operation. Tel +96277414388. email :- [emailprotected] P.O.POX:-Jordan-Mdaba 11710-986 Introduction: Huggins and Hodges (1941) described the androgen dependent nature of prostate cancer by the observation that surgical castration resulted in prompt relief of pain in patients with bone metastatic prostate cancer, and since that time hormonal manipulation in the treatment of prostate cancer has evolved(1,2, 3). Prostate cancer is the most frequent visceral malignancy and the second leading cause of death in American men. It has been estimated that approximately 184 500 new cases will be diagnosed and over 39 200 men will die from prostate cancer in the United States in 1998 (3, 4, 5, 6). The annual Medicare expenditure for prostate cancer is approaching $1.5 billion, of which a large portion is spent on androgen deprivation therapy. Androgen deprivation therapy can be achieved medically using luteinizing hormone releasing hormone (LH-RH) agonist or surgically by bilateral orchiectomy. While the two approaches have similar efficacy, medical therapy is significantly more expensive than surgical therapy (4, 7, 8). The trend towards day case surgery in many countries is increasing (9); it is an efficient way of using resources and reducing waiting lists. Intrascrotal operations are particularly suitable for day case surgery (2, 3, 7, 9). Regional block techniques have been used for minor urological procedures and one such technique is spermatic cord block (2, 3, 7, 9). This is a simple, cost-effective technique suitable for adults undergoing intrascrotal surgery. It is particularly appropriate when the patient is considered a poor risk for general anaesthesia (9, 10, 11, 12, 13). We report our successful experience with local anaesthesia for a series of 96 patients undergoing a bilateral Subcapsular orchiectomy in Prince Hussein Urology Center Materials and Methods: Of 96 patients from April 2004 October2008 who were diagnosed prostatic cancer. They were nonlocalized prostatic cancer or physical status not suitable for radical prostatectomy surgery. We excluded patients who were allergic to bupivacaine hydrochloride, or having severe hypertension, recent MI, unstable angina, uncorrected bleeding disorder, paraplegia and neuro- sensory deficit. During the pre-operation we explained to the patient the procedure and provided anesthesia only on the scrotal content and scrotal skin at the incision site; he would feel some pain initially during the injection of anesthetic agent, and he might have some abdominal discomfort during the cord manipulation, and postoperatively he could ambulate immediately .The patient was not allowed to take anything by mouth after midnight before the procedure. Every patient was given an intravenous line and an anaesthetist was on stand-by to give anesthesia if spermatic cord block did not work. The scrotum is prepared by pre-operative shaving and is cleansed using 10%povidone -iodine solution and draped in sterile fashion .The anaesthetic agent is a mixture of 1%lidocaine and 0.25 % bupivacaine hydrochloride was selected, the patient was in supine position. The pubic tubercle is palpated; the cord was trapped between the index and middle fingers of the surgeon; 1 cm below and medial to the tubercle was the injected point, infiltrate at skin and pass the needle vertically down to the anterior aspect of the pubic bone. In it course the needle, thus passes through the spermatic cord, 8 -10 ml of anesthetic solution is injected through the cord at slightly different angle and the needle entering the blood vessel be aware of. The instilled volume of anaesthetic solution causes visual ballooning of the grasped segment of the spermatic cord; this bulge is then gently squeezed between the thumb and index finger to disperse the anesthetic fluid within the spermatic cord. After the spermatic cord was blocked the skin at the incision site was infiltrated with 3-5 ml anesthetic fluid, 3 -5 minutes before the start of the operation so that drug became effective. Orchiectomy was performed in the midline raphae incision with epididymis sparing fashion to create a round structure mimic a small testis for cosmetic result. A longitudinal incision is made through the tunica albuginea of the testis along its free border, exposing the seminiferous tubules. The internal contents of the testis are quickly freed from the side walls by gentle squeezing the outside of the capsule. This is the most sensitive part of the procedure but if discomfort is experienced, more anesthetic fluid can be injected directly into the cord. The tubules can be disconnected at the testicular hilum using scissors. Any tissue remaining on the inside of the capsul e is removed and meticulous haemostasis is established by diathermy. The capsule is resutured with a continuous layer of 3 vicryl. The procedure is repeated on other side through the same skin incision and the wound closed using 3 vicryl to the tunica vaginalis and covering layers, and 4 subcuticular dexon to the scrotal skin. The procedure is completed by local dressing, a large gauze pressure pad and a scrotal support to prevent haematoma formation. During the operation, the patient was monitored and blood pressure, pulse rate and abnormal symptom were recorded; when surgery finished the patients pain score of the procedure (including pain of anaesthetic injection) was assessed immediately by using visual analog pain scale (0 = no pain, 5 = moderate pain and 10 = worst possible pain) . At 1-week follow-up, the patients symptom and wound were evaluated again. Discussion: An LHRH agonist is the preferred first option to treat patients with advanced prostatic cancer. However, clinical studies have suggested that an orchiectomy is superior to an LHRH agonist in that it more rapidly achieves castrate levels of testosterone, avoids the testosterone flare, is less expensive, and has superior therapeutic compliance (1, 8, 10, 14). If there were a castration procedure that did not adversely affect life satisfaction and the male image, this option might become more frequently recommended and chosen. Several attempts have been made to achieve this goal. In 1942, Riba pioneered the subcapsular orchiectomy, a procedure that involved the removal of the testicular parenchyma and the simple closure of the tunica albuginea (10, 13, 16, 17, 19). No difference was observed between patients who underwent a bilateral total orchiectomy and a subcapsular orchiectomy in preoperative and postoperative testosterone or luteinizing hormone levels( 5,17,19).Most importantly, serum PSA and 3-year survivals for patients undergoing a bilateral total orchiectomy and a subcapsular orchiectomy were determined to be similar (3,10). The technique of spermatic cord block is base on the anatomy 🙠 2, 3, 4, 18) .as the cord emerges from the external ring, it passes over the pubic tubercle and the shifted medially to the scrotum. In this region it is closely associated with the ilioinguinal nerve and the genital branch of genitofemoral nerve, which supply the testis and its covering, the epididymis and the vas deferens but not the scrotal skin. The scrotal skin receives sensory supply from the pudendal nerve and the perineal branch of the posterior cutaneous nerve of the thigh; therefore it needs to be infiltrated with the anesthetic agent separately from spermatic cord block (5, 9, 11, 12). Good result of spermatic cord block facilitates a successful orchiectomy. No complication related to anesthesia was detected in the series. The advantage of spermatic cord block is its short time of recover, low cost and may be performed in patient who has high risk of anesthesia (7, 11, 14, 18). 10 patients numbered thei r visual analog pain scale 10. Five had underlying anxiety disorder, while the other 5 one had severe pain that needed to be converted to general anesthesia which might have caused by his obesity (BW 86.5 kg, HT 165 cm, BMI 31.77 kg/m2; mean BW = 62.55 kg; patients who had success operation whose BW was in the range of 45 68 kg). other 4 patients have huge inguinal hernias that also make procedure more difficult .Obesity made it difficult to palpate the cord and inject anesthetic agent to the correct point, so the spermatic cord block did not work well. Three patients had bradycardia (pulse rate = 50|min. 49|min.54/ min) which might due to his vagovagal reflex when the cord was under traction; however they developed no other symptom or hypotension. Intrascrotal procedures can be performed easily with spermatic cord block rather than general anaesthesia. This offers advantages to both the patient and the treating hospital. For the patients the length of time spent in the recovery room, the chances of intraoperative anesthetic complications and the need for postoperative analgesia are all reduced. For the hospital the obvious advantages in terms of bed occupancy and cost saving may be realized (5, 9, 11, 12) We evaluated the cost-effectiveness of androgen suppression strategies for men with advanced prostate cancer. Our principal finding is that the effectiveness of orchiectomy is much less expensive. The subcapsular technique bypasses the need for prosthesis thus contributing to a lower cost when compared to total orchiectomy. Result: Of the 96 patients age 65 83 yr (mean =71.11 yr), operative time 20 55 min (mean 36.00 min), amount of anesthetic mixture 10 30 ml (mean = 20 ml) orchiectomy under spermatic cord block were successful in 91 96 (94.79 %). Five patients failed because they had so severe pain that needed to be converted to general anesthesia. Three patients had bradycardia (pulse rate = 50|min. 49|min.54/ min), 2 patients had tachycardia (pulse rate = 124/min, 102/min). None of patients had hypotension, nausea or vomiting. No complication related to the anesthesia nor the procedure was seen. Most of the patients felt little pain especially when monopolar electrocautery was used to cut the tissue or stop bleeding. Post-operatively, all of the patients ambulated immediately; 86 patients (89.47 %) rated their visual analog pain scale between 0 6; 10 patients (10.42%) numbered their visual analog pain 10 (5 of them converted from local to general anesthesia). When classify to mild (pain score 0-3/10), m oderate (pain score 4-6/10), and severe pain (pain score 7-10/10). 59 patients (61. 46 %) were in mild pain group, 27 patients (28.13 %) had moderate pain and severe pain in 10 patients (10.42 %) table-1. At 1-week follow-up, 2 patients suffer from surgical wound infection , one is admitted to hospital for dressing and debridement, the other treated as outpatient with wound dressing and oral antibiotic treatment; 5 patients had scrotal hematoma which improved with time and conservative treatment. Conclusion: Bilateral subcapsular orchiectomy is safely done under local anaesthesia, simple and coast effective. For preoperation, the patient needs to be explained the procedure and some symptoms that he may experience during the operation. Spermatic cord block is not suitable in patient with anxiety or obesity. They should receive general or spinal anesthesia.
